HealthReach Community Health Centers is a system of eleven Federally Qualified Health Centers in Central and Western Maine. A private, non-profit organization with a 43-year history, HealthReach is funded by patient fees, grants and individual donations. We provide primary care services for the whole family to nearly 28,000 Maine residents and travelers each year - regardless of insurance status or ability to pay. We strive to be the provider, employer, and healthcare partner of choice in each of the communities we serve.
Great Family Health Care
Our healthcare providers offer outpatient primary care and preventive services to people of all ages. All of our practices have achieved National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) Recognition and offer accessible, high quality healthcare with integrated behavioral health services focused on the patient’s care experience. HealthReach providers treat the “whole person” - not just physical symptoms, and form partnerships with their patients, encouraging them to take an active role in their care.
Affordable
HealthReach services are available to everyone, regardless of ability to pay. We accept Medicare, MaineCare (Medicaid) and private insurance. Through funding from the U.S. Bureau of Primary Health Care, we also offer a sliding fee program to uninsured and underinsured residents to assist with the cost of health center services. In addition, our Connectors offer application assistance for programs that help with the cost of healthcare and medications including the Health Insurance Marketplace.
Close to Home
HealthReach serves families in over 80 rural communities, primarily in Kennebec, Franklin, Oxford, Somerset, Lincoln, Sagadahoc and Androscoggin counties. Nearly 28,000 people chose us as their primary care provider in 2017, totaling over 97,000 visits. We work closely with four hospitals located in our service areas: Franklin Memorial Hospital (Farmington), MaineGeneral Medical Center-Alfond Center for Health (Augusta), Redington-Fairview General Hospital (Skowhegan) and Stephens Memorial Hospital (Norway). We also collaborate with local organizations, state agencies and individuals who deliver crisis, health, psychosocial, education and other complementary services to area residents.

HealthReach provides comprehensive primary and preventive care services to children and adults, including care of acute and chronic illnesses, checkups, school and sports physicals, immunizations, family planning, and lab services.
